MOH and NHG Health visit to CHP Health Post at Blossom Seeds
Blossom Seeds was delighted to host colleagues from MOH and NHG Health at our Active Ageing Centre @ East Canberra for a Community Health Post (CHP) showcase.
The visit highlighted how community and healthcare partners work together to bring preventive care and support closer to seniors.
They also spent some time to interact with our seniors at Gym Tonic and communal dining, gaining insights into seniors’ needs and community care in action. Thank you for supporting our seniors to age well and purposefully in their community
