Building the Future of Integrated Community Care Providers (ICCP)

𝐁𝐮𝐢𝐥𝐝𝐢𝐧𝐠 𝐭𝐡𝐞 𝐅𝐮𝐭𝐮𝐫𝐞 𝐨𝐟 𝐈𝐧𝐭𝐞𝐠𝐫𝐚𝐭𝐞𝐝 𝐂𝐨𝐦𝐦𝐮𝐧𝐢𝐭𝐲 𝐂𝐚𝐫𝐞 𝐏𝐫𝐨𝐯𝐢𝐝𝐞𝐫𝐬 (ICCP)

Nearly 𝟗𝟎 𝐂𝐨𝐦𝐦𝐮𝐧𝐢𝐭𝐲 𝐂𝐚𝐫𝐞 𝐎𝐫𝐠𝐚𝐧𝐢𝐬𝐚𝐭𝐢𝐨𝐧𝐬 came together at𝐌𝐁𝐒 𝐟𝐨𝐫 𝐭𝐡𝐞 𝐈𝐂𝐂𝐏 𝐒𝐞𝐜𝐭𝐨𝐫 𝐄𝐧𝐠𝐚𝐠𝐞𝐦𝐞𝐧𝐭 𝐒𝐞𝐬𝐬𝐢𝐨𝐧. A meaningful milestone in our shared journey to support seniors with higher care needs living in the community.

The session celebrated progress in strengthening ICCP partnerships, highlighted key achievements from Phase 1, shared plans for Phase 2, and looked ahead beyond.

It began with an opening address by 𝐀𝐈𝐂 𝐂𝐄𝐎 𝐌𝐫 𝐓𝐚𝐧 𝐂𝐡𝐞𝐞 𝐖𝐞𝐞 Agency for Integrated Care - AIC Singapore, followed by a progress update from the 𝐀𝐈𝐂 𝐥𝐞𝐚𝐝𝐞𝐫𝐬𝐡𝐢𝐩 𝐭𝐞𝐚𝐦 and rich 𝐫𝐨𝐮𝐧𝐝𝐭𝐚𝐛𝐥𝐞 𝐝𝐢𝐬𝐜𝐮𝐬𝐬𝐢𝐨𝐧𝐬 shaping the future of community care.

Heartfelt thanks to 𝐀𝐈𝐂 𝐚𝐧𝐝 𝐀𝐏𝐎 Ageing Planning Office for listening to sector concerns and addressing real challenges on the ground.
